Name and Address Form
You may change your address on-line using
the Employee’s Self Service intranet web-site or you
may download the form and send it to the Office of Payroll
for processing. NOTE: to change your name, download and
complete the form and send it to the Office of Payroll.
A copy of your new social security card MUST accompany the
Change of Name and Address Form when requesting a name change.
- Combined IRS’ Form W4 and
MD Form MW 507
This form combines the IRS’ Form W4
and the Maryland Form MW507, “Employee Withholding
Allowance/Exemption Certificate,” into one form. This
combined form should be used to: change your marital tax
status; the number of exemptions claimed; claim exemption
from income tax withholding; or change the amount of additional
federal or state income tax withheld. If an employee does
not provide an original, complete, valid, signed form, then
the employer must withhold income tax as if the employee
were single with zero exemptions (the maximum amount of
tax will be withheld). A claim of exemption from income
tax withholding is effective for one year and a new form
attesting to the exempt status must be filed by February
15. Download and complete the form, and send it to the Office
of Payroll, Timonium.

for Direct Deposit
ALL EMPLOYEES, including substitutes, paid helpers, and
contractual employees, ARE ELIGIBLE FOR DIRECT DEPOSIT.

- SEMS Substitute Correction Form
Use this form to report any adjustments to time worked by SUBSTITUTE administrators, teachers, or nurses previously reported in the SEMS.
- SEMS Time & Leave Correction Form
Use the TLC Form to report any adjustments to previously reported SEMS absences. Approved absences should be reported in TEN MINUTE increments (.1 = 10 minutes; .2 = 20 minutes; .3 = 30 minutes, etc.).
